Co-Curricular Forms

All forms will be completed electronically when you do the online registration. NO paper forms should be turned into the office. You will check off necessary documents OR upload the documents needed through the online registration.

WIAA Physical Form

Each student athlete that intends to tryout and participate in any Forest Park athletic offering must have a current physical form completed by both the parent and physician clearing them to participate in sports. Without this form, students will not be able to participate, even in tryouts. This form will be uploaded during your online registration. Physical forms will not be accepted in the office.

Physical exams by a licensed physician are required in order to participate in school-sponsored athletics programs. Please alert administration if your student's health or physical abilities are limited in any way in between physical exams.

Physical exams that take place on April 1 and thereafter are valid for the following two full school years; physical examinations that take place before April 1st are valid only for the remainder of the current school year and the next year.

Request and Release for Private Transportation**Please fill out the private transportation form if you student will be picked up from sporting/club events instead of taking the provided transportation. Please contact the coach/advisor for information on when this form is necessary.
